Please make your booking online through the website. I am using a 3rd party booking system ‘Sports Injury Fix’ to manage bookings and take secure payments. This booking system allows you to select a convenient appointment time and make a payment prior to your appointment. I have introduced this measure to minimise cash handling in light of COVID-19. Upon booking you will be emailed a consultation form for your first appointment. Please complete this in as much detail as possible prior to your booking. For all future appointments (if there are any changes to your health, contact details or personal circumstances) please let me know, so I can update my records accordingly.

When you book an appointment with Plymouth Injury Clinic you are agreeing to our Terms & Conditions and Privacy Notice. You are also agreeing to attend the appointment you have booked at the time and date confirmed.

Appointments

In the event of you presenting with COVID-19 symptoms or you have a long-term medical condition / illness which varies from day to day I will use my discretion to discuss cost implications on a case by case basis.

Current appointment times are 60 minutes (1 hour) this includes the time it takes for an initial assessment. It is important that you arrive for your appointment at the arranged time. Please do not arrive early during the pandemic as I will not be able to see you earlier than scheduled. This is due to the requirement of: deep cleaning premises between each appointment, changing of PPE /uniform/towels and hand washing.

If you arrive late for your appointment, provided it is no later than 15 minutes I will still be able to provide your session. However please be aware for the reasons stated above in relation to COVID-19, I am unable to overrun on your appointment to make up for lost time.

Confidentiality
All client information will be kept confidentially by Plymouth Injury Clinic  unless I am required to disclose information by law. I will not share, sell, discuss or distribute client information and all records will be stored safely.

Personal information collected at the initial consultation will remain private, however in the exception whereby you wish me to share this with a 3rd party, such as in the event of requiring a referral I will only do so with your full consent.

To ensure there is no unauthorised access to your information, accidental loss or damage the clinic/gym is locked and alarmed when it is vacated . There are also security cameras monitoring the external corridors of the premises. Furthermore, your information will either be stored in a lockable cabinet or an online booking system which is password protected. I will retain your information for the time period in accordance with UK law. Destruction of personal information will occur after this time period has lapsed and will be done in accordance with general data protection regulation (GDRP) guidelines.

Staff of Plymouth Injury Clinic will only have access to your personal information during their employment within the gym/clinic and with the sole purpose of providing you with the most appropriate treatment and care. Physical copies of your personal information will remain securely on the premises unless with your consent I am required to forward it to a 3rd party.

Health & Safety
Staff of Plymouth Injury Clinic hold current First Aid Certification, are DBS checked and have professional and public indemnity insurance for the provision of personal training, sports rehabilitation, sports therapy and sports massage. I am a member of the Sports Therapy Organisation (STO) and adhere to their guidelines in relation to continuing professional development (CPD), conduct and ethics.

If I am feeling unwell, suffering with a virus, infection or general health complaint, I will be unable to carry out your session due to the risk of cross-contamination or infection. I will contact you at the earliest opportunity to inform you, and re-arrange your appointment or give a refund where applicable.

If you are feeling unwell with either a cold, virus, infection or general health illness (to prevent risk of cross contamination or infection) I request that you do not attend your appointment. In these circumstances please contact me at info@plymouthinjuryclinic.co.uk or Tel: 07932 026176 and I will apply the cancellation policy.  In the current climate it is not acceptable to attend appointments if you are suffering with any COVID-19 symptoms. For more information on the COVID-19 policy please read the section below.

Plymouth Injury Clinic and its staff will not be held accountable for the loss or damage of your personal belongings during your time in the gym/clinic. Storage boxes are provided during your treatment, and I recommend that you use them to keep your personal items in one place.

Plymouth Injury Clinic reserve to right to refuse treatment and provide services related to exercise or therapy if clients are physically or verbally abusive towards staff. Offensive language, inappropriate behaviour, racism, sexism and all forms of discrimination will not be tolerated on the premises and such behaviour will result in termination of your treatment and a permanent ban. My expectation is that clients come to their appointment clean and hygienic; I will not provide your session if this standard is not met.

COVID-19
Due to the COVID-19 pandemic there will be an additional declaration form to complete stating that:

The client has not been in contact with anyone with Covid-19 in the last 14 days, to the best of their knowledge

The client has not had any of the following symptoms:

  • a high temperature – this means you feel hot to touch on your chest or back (you do not need to measure your temperature).
 
  • a new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual).
 
  • a loss or change to your sense of smell or taste – this means you’ve noticed you cannot smell or taste anything, or things smell or taste different to normal.
 

Should the client contract the virus, they must inform the practitioner/trainer as soon as possible. Plymouth Injury Clinic are obliged to inform NHS Test & Trace in the event of this occurring.

I will be taking the temperature of each client with an infra-red thermometer prior to all sessions. This will be recorded & stored inline with GDRP guidelines. There will also be a signed declaration from the practitioner/trainer stating I am free from COVID-19 symptoms and that I have not (to the best of knowledge) been in contact with anyone who has either tested positive or had symptoms.

To minimise face-to-face contact please include as much information regarding your condition in the consultation form provided. Free no obligation telephone/video calls are available if applicable to discuss your condition prior to making an appointment. 

Disclaimer
Plymouth Injury Clinic provide injury prevention, diagnosis, treatment, rehabilitation personal training and offer nutritional guidance, using a variety of techniques and treatment modalities. I am fully insured and qualified to provide all the aforementioned services. In the event of you presenting with a condition that I am unable to diagnose and /or treat, or whereby my treatment/s may make your condition worse I will (with your permission) offer referral or request you visit your general practitioner for onward referral.

Clients have a duty to provide accurate and true medical and personal information to the best of their knowledge. Furthermore they are required to keep Plymouth Injury Clinic up to date with any changes to medication, symptoms, medical concerns or treatments they are having investigated or undergoing treatment for.

Staff at Plymouth Injury Clinic may refuse to treat any client or part of their body with just and reasonable cause. Plymouth Injury Clinic reserves the right to terminate any appointment at any time if they believe that it is not safe or in the client’s best interest to continue. Likewise, the client has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.
